According to the 2000 National Demographic Study, the average age of Holocaust survivors is 75 years and 13% live in the South Florida area. The average age of South Florida holocaust survivors is 84. South Florida is home to approximately 18,000 survivors according to Dr. Ira M. Sheskin, Associate Professor, University of Miami, Dept. of Geography and Regional Studies. Because Holocaust survivors suffer extreme problems when placed in nursing homes and assisted living facilities, greater efforts need to be taken to allow them to stay at home. Many of the age-related issues survivors deal with are faced by every senior citizen; but research is showing Holocaust survivors have a more difficult time due to early-life traumas with social workers seeing symptoms similar to post-traumatic stress disorder including bad dreams, insomnia and thoughts of suicide. Institutionalization exacerbates these problems. Survivors that did not require assistance previously are becoming increasingly frail and unable to manage their activities of daily living as easily as before and those that are receiving our help will require a more intensive level of care as they age. Every effort must be made to allow Holocaust survivors to remain in their homes as long as possible. |

Project Description: |
The services provided by the four Jewish Family Service agencies in the three counties are basically the same. Each individual is assigned a case manager who does an assessment and develops a service plan that is contracted with appropriate home health care vendors. Vendor�s services include personal care (bathing and dressing), housekeeping, meal preparation, laundry, shopping, and transportation to medical appointments. Case managers visit clients regularly to review and assess the client�s status, making changes in care plans as required. In addition to homecare and case management, survivors also require and receive screening and assessment, mental health counseling, food pantry supplements, kosher meals, telephone reassurance and socialization activities. Most of the Holocaust survivor population does not receive government means tested assistance of any kind. They do receive social security, Medicare and may receive modest reparations. Holocaust Survivors in South Florida receive an average of 5 hours home healthcare per week, far below the 12 hours per week recommended by the National Association of Jewish Family and Children�s Agencies |

Measurable Outcome Anticipated: |
There is a 90% satisfaction rate. Holocaust survivors will receive the in home services that they need based on the results of the needs assessment performed on each client by the case manager. Clients are surveyed after they begin to receive services to evaluate how effective the services are and measured to determine if the appropriate services are being met. The provision of these services will be to delay for at least 90% of the clients served, if not eliminate, the need for the client to enter an assisted living facility or nursing home. |
