Behaviors and lifestyles that lead to chronic disabling diseases. Florida continues to rank in the bottom tier of states (#41 out of 50) listed in America's Health Rankings� (2006 and 2007). This places the state below the national rate of improvement and in danger of slipping further behind in healthiness compared to the nation as a whole.

Project Description:
Bethune-Cookman University proposes to engage high health risk citizens, their families and communities in a broad-based, collaborative community health improvement program. The program focus will be to better understand and alter behaviors and practices that lead to chronic diseases. Using the resources and services of it's new Wellness Center, the University�s School of Nursing will recruit, specifically train and mobilize a cadre of �healthy lifestyle assistants or health coaches,� to educate, mentor and monitor Volusia County citizens and families in the neighborhoods and communities at greatest risk to make healthy life choices that will help reduce the incidence and impact of chronic disabling diseases and better manage existing conditions through better awareness of the signs, risk factors and symptoms of these diseases.

Measurable Outcome Anticipated:
It is anticipated that the project will: 1) Identify a significant number of uninsured, low-income and/or minority citizens and families throughout target area who are disproportionately affected and are least likely to receive the proposed interventions, and engage them in developing preventive behaviors necessary to reduce their risk; 2) Identify and engage a significant representation of local, regional and state health-related agencies and providers to actively collaborate with the program; 3) Help increase the number among the target group who engage in routine preventive health maintenance under a physician�s care; 4) Help increase the number among the target group who show evidence of modifying risk factors. Project staff will also monitor the economic impact of these efforts on direct health care costs for people with chronic conditions by assessing the decrease in the number of hospitalizations among the target group and in new cases of disease identified during each quarter of the project, was well as the decrease in use of emergency services by citizens and families engaged in the program for chronic disease related symptoms and conditions.
