Project Description: |
The four Specialized Alzheimer�s Day Care Centers, established between 2003 and 2008, will serve 450 Alzheimer�s disease and related disorders patients and caregivers annually. Each center operates five days a week, between the hours of 7:30 a.m. to 5:30 p.m. These hours have proven effective in meeting the needs of our working caregivers (45% of our caregiver population). Each Specialized Alzheimer�s Day Care Center maintains a 1:5 staff to patient ratio. Often, the use of volunteers further reduces that ratio. Approximately 15 volunteers are trained to work at the Specialized Alzheimer�s Day Care Centers. Each Specialized Alzheimer�s Day Care Center provides a wide array of therapeutic activities specifically designed to meet the unique cognitive, physical, social and spiritual needs of the patients served. The therapeutic activity programming (inclusive of nutritious meals and snacks) serves as an invaluable treatment modality for patients affected by Alzheimer�s disease or a related disorder. Each Specialized Alzheimer�s Day Care Center employs a minimum of one full time licensed nurse (LPN) which ensures patient stabilization and longevity through consistent interventions such as medication administration and monitoring, management of co-morbid illnesses, regular monitoring of vital signs, comprehensive individualized patient care planning and ongoing communication with each patient�s physician. Additionally, disaster preparedness and hurricane planning for both patients and caregivers serves as an integral component of case management for those participating in the Specialized Alzheimer�s Day Care Centers. The large majority of Alzheimer�s Community Care�s Specialized Alzheimer�s Day Care Centers operate in space provided through progressive collaborations with faith based organizations in the local community. This community based model of partnership has proven to be extremely cost effective and allows Alzheimer�s Community Care to ensure that precious financial resources are utilized for direct service care and affords us to work in culturally diverse communities with great financial disparity. Alzheimer�s Community Care operates weekly support groups in each Specialized Alzheimer�s Day Care Center service area providing caregivers with much needed support, education and training. Additionally, Alzheimer�s Community Care provides immediate access to nurse case management services, information and resources via an Alzheimer�s Crisis Response Service that operates twenty-four hours a day, seven days a week. All funds awarded as a result of this request will be used to provide services to Alzheimer�s disease and related disorders patients and their caregivers. |

Measurable Outcome Anticipated: |
To lower institutionalization of patients by two to four years; lower the risk of the mortality of caregivers from 63% to approximately 40% or lower; lower the risk of hospitalization for caregivers. Approximately 13% of patients annually (identified by our evidence-driven data) are at risk of discharge from the day service sites, however 87% will not enter a nursing home-a savings of institutional dollars totaling $10,450,351.50 ($55,001.85 Medicaid x 190 patients) to caregivers, community and society. |
