Project Description:
The Jay Weiss Pilot Program to Address Health Disparities in Overtown will improve the health status and quality of life of severely ill and underserved individuals through home-based health promotion and harm reduction reinforced by community health promoters. The pilot will show that intensive education and support will improve acccess to and adherence with medical care, reducing complications from chronic disease and decreasing ER visits and hospitalizations. By comprehensive approach to chronic disease management in the most difficult patients, we expect to see dramatic cost savings to the health care system. This program will also aid community development by seeking to recruit and employ individuals from the area, providing stable high-quality jobs, and by developing and supporting prevention, education and hard reduction activities.

Improved health status for chronically ill and underserved individuals in the Overtown area and reduced number of ER visits and hospitalizations, with a goal of saving approximately $15,000 per patient per year to the public health system.
