Statewide Interest: |
St. Catherine's Rehabilitation Hospital (SCRH), has developed a multi-disciplinary assessment clinic for individuals diagnosed with Parkinson's Disease. This clinic addresses a statewide interest pursuant to F.S. Chapter 216.052(1). The service is designed to keep Floridians who have Parkinson's Disease in the community and living at home, thereby decreasing the state's cost in caring for these individuals since they will not be utilizing state Medicaid funds for long-term nursing home care. The incidence and prevalence of this disease is increasing as our longevity is also increasing. Florida has a disproportionate number of people affected by Parkinson's Disease since the state has the highest number of elderly (>65 years old) in the United States. The U.S. Dept. of Health and Human Services has published an article recommending that states develop partnerships and coalitions, focus on assessment and evaluation, and promote professional education in dealing with chronic conditions such as Parkinson's. |

� Prevention of costs associated with hospitalization: In 1999 and 2000 there was an average of 238,400 hospital discharges per year, with a mention of Parkinson's disease (from CDC-NCHS). A multidisciplinary team assessment can identify potential health concerns before they occur via risk factor assessment and thorough diagnostic testing. With the appropriate medical management of identified problems, declining function and its associated costs can be minimized.
� Prevention of costs associated with institutionalization: In 1999 approximately 9 percent of men and 4.3 percent of women over 65 years of age had Parkinson's disease upon admission to a nursing home (from CDC-NCHS). It is clear that many people with Parkinson's Disease become institutionalized. The multidisciplinary team in the clinic will focus on maintaining independence, including promoting the safest living situation which uses the fewest resources (e.g., Medicaid funds). Individuals who do not receive this type of aggressive intervention designed to maintain independent function and living would likely end up occupying a nursing home bed or becoming a burden on family and caregivers.
� Decreased burden on caregivers for appointments and consultations: The clinic will allow patients (and caregivers) the luxury of having all applicable medical disciplines (MD, RN, OT, PT, SLP, LCSW) available for consultation at one site, and on one day. This significantly reduces the burden of care involved with keeping multiple medical appointments. Overall quality of life is improved for both the client and the caregiver.
� Maximizing use of State and Federal resources already in place to assist qualifying citizens/residents: The state of Florida already has a number of programs in place to assist its citizens and residents who are physically challenged (e.g., PACE). The Social Worker on the clinic's team will keep abreast of all existing state and federal programs available to assist the persons served. This assures that Florida residents and citizens are utilizing available resources, thereby maximizing cost/benefit of these resources.
