Project Description: |
To be piloted in Pinellas County, Florida, CCIP will allow the maintenance of delinquent youth safely in their home communities, and provide enhanced support for youth returning from commitment programs in terms of successfully reintegrating into their families, schools, and communities. These evidence-based services will effectively address behavioral, social, mental health and substance abuse issues of the youth and their caregivers. The array of services will also strengthen youth and/or family skills in communication, non-violent problem resolution, and behavior management. Key program components include:
A. Assessment and Treatment Planning: In collaboration with the Department of Juvenile Justice, precursors to CCIP rollout will include the development of specific admissions criteria to decrease the chances of referral of youth to the program who cannot be safely served in the community.
B. Community-Based Services Array: The program will make an array of services delivered by a host of community-based providers available to the youth and family based upon initial and ongoing assessments. Evidenced-based program models utilized include Multisystemic Therapy; Functional Family Therapy; and Multidimensional Treatment Foster Care. Less intensive services will include: Mentoring/Tutoring, Vocational and Educational Support, and Life Skill/Independent Living Skill Services;
C. Case Management/Case Monitoring: Includes coordination with juvenile services, court, and other community based representatives; progress reports and termination summaries; maintaining youth records and case notes; family linkages; transition support; and performance monitoring
