Statewide Interest: |
The program provides a medical home delivering comprehensive medical and transition services to youth and young adults with physical or developmental disabilities who are facing the loss of pediatric primary and specialty care. There are an estimated 2500 youth in Duval, Baker, Clay, Nassau and St. Johns counties with disabilities who can benefit from these services. A substantial number of community providers provide support for this program. |

Provide a health care transition program, including primary care, referral to adult specialty physicians, transition planning services, health care planning, coordination with educational/vocational and independent living transition services and assistance securing health insurance or other financial benefits. |

Measurable Outcome Anticipated: |
1.Provision of a Medical Home for all youth and young adults with chronically disabling medical or developmental conditions who are residents of the five county area; 2. Reduction in preventable emergency room visits and hospital admissions; 3. Development, testing and evaluation of a model system for transition that can be replicated throughout the state; 4. Increase in the number of individuals with chronic disabling medical and developmental conditions that are transitioned to the adult health care system and gainfully employed. |
