Project Description: |
The GulfCost-Florida Homeless Continuum of Care refers to programs and services developed and implemented for homeless adults (with focus on veterans) that include; 1) Substance Abuse Outpatient Treatment Services, 2) Mental Health Treatment & Counseling (group/individual), 3) Assessment and Case Management Services, 4) Family Support Outpatient Groups and 5) Emergency Shelter and Inebriate Receiving Services, 6) Referrals for community-based services needed for community re-intergration WestCare's objective is to increase veterans services to 20 new residential beds and 100 outpatient slots for veterans with WestCare's current residential facility. |

Provide veterans services at a rate of 20 residential beds and 100 outpatient slots annually. Provision of Residential & Outpatient Substance Abuse, Mental Health & Intervention Treatment Services will result in more that 150 veterans successfully transitioning into the community with sustainable employment and life skills annually. |
