Health Insurance/Payment of Benefits/Claims Forms [CPSC]; Requires that insurer make payment to designated provider of services whenever an insured, using any health insurance claim form, specifically authorizes payment of benefits directly to any recognized hospital, licensed ambulance provider, physician, dentist, or other person who provided services in accordance with provisions of the policy. Provides that insurance contract may not prohibit payment of benefits directly to such providers, etc.
