Name Changes; requires petitioner for change of name to submit fingerprints to clerk of court; provides for fee; prohibits convicted felon from filing petition for change of name while incarcerated; provides exception for victims & witnesses certified for protection or relocation; requires certain forms to include category for all names used by person. Amends 68.07.
