Health Care; provides for statewide provider-qualification dispute-resolution program to be established by AHCA; authorizes agency to adopt rules; provides terms & conditions of contracts between health insurer & health care provider; eliminates requirement for 60 days' written notice of cancellation without cause of contract between HMO & health care provider, etc. Creates 408.7058; amends 627.6474, 641.315.
