Managed Care/Health Care; prohibits termination of provider contract except under certain circumstances; allows patients to disenroll in certain plans & reenroll in other plans under certain circumstances; prohibits HMO from advertising availability of certain providers under certain circumstances; provides exception. Amends 641.315,.3903.
