Patient Records; requires disclosure by licensed facility concerning individually identifiable health information transmitted to site outside United States; requires notice to & consent of patient; provides for renewal & revocation of consent; provides for consent form & contents thereof; prohibits discrimination based on refusal to grant consent. Amends 395.3025.
