Health Care Services; specifies conditions under which health care provider must be permitted to participate as service provider under health plan offered by managed care organization; defines term "managed care organization"; requires that said provider be reimbursed for providing services under specified conditions; provides for civil penalties; provides for construction of policies; provides for application. Amends 627.419.
