Dentistry; requires dentist to provide specified info. to patient re restorative or cosmetic cases; provides that requirements re certain dental work apply to registered dental laboratories in lieu of "any unlicensed person"; revises procedure by which dentist may outsource certain dental work; requires registered dental laboratories to ship or deliver cases or materials to dental offices in specified bag, etc. Amends 466.018,.021,.032,.036.
