Transitional Services/Young Adults/Disabilities; Creates Health Care Transition Services Task Force for Youth and Young Adults with Disabilities within DOH to assess need for health care transition services for youth and young adults with disabilities, develop strategies to ensure successful transition from pediatric to adult health care system, & identify existing & potential funding sources; provides for expiration of task force.
