Transitional Services/Young Adults/Disabilities [SPCC]; Creates the Health Care Transition Services Task Force for Youth and Young Adults with Disabilities within the Department of Health to assess the need for health care transition services for youth with disabilities, develop strategies to ensure a successful transition from the pediatric to the adult health care system, and identify existing and potential funding sources, etc.
