Domestic Partnerships [SPSC]; Requires the DOH to create and distribute the Declaration of Domestic Partnership and Certificate of Registered Domestic Partnership forms to each clerk of the circuit court. Describes the required contents of the declaration. Requires two individuals who wish to become partners in a domestic partnership to complete and file a declaration form with the clerk of the circuit court, etc.
