Insurance Coverage for Prescription Drugs; Prohibits limiting, reducing, or denying prescription drug coverage under health insurance policies or health maintenance contracts for individuals under specified circumstances; preserves insurer or HMO authority to make specified uniform changes; requires insurers & HMO's to notify insureds of such changes; specifies nonapplication to Medicaid managed care plans.
