Assisted Living Facilities; Specifies conditions for relocation or termination of residency in assisted living facility's resident bill of rights; requires administrator or designated employee of facility to sign notice of relocation or termination of residency & requires physician's signature; requires licensee to provide written notice to resident & other specified persons prior to relocation or termination of residency, etc.
