Nursing Home Diversion Program; Directs AHCA to expand nursing home diversion program to include Medicaid recipients who meet certain criteria; specifies locations for phased-in implementation of program; revises conditions for enrollment in program; provides for Medicaid recipient choice with regard to contractors; requires nursing home diversion contractor to provide an enrollee with information regarding alternative service providers; requires certain enrollees to participate in program; requires program to combine funding for Medicaid services provided to specified individuals, etc.
