Children Who Are Deaf or Hard of Hearing; Requiring health care providers to provide an opportunity for a child’s parent or legal guardian to provide contact information so that he or she may receive information from specified service providers when the hearing loss is identified; requiring the Department of Health to register certain service providers and institutions; allowing a parent or legal guardian to request services from a participating service provider; providing that the level of services received is based on the child’s individualized education program or individual and family service plan, etc.
