Physician Orders for Life-Sustaining Treatment; Requires DOH to develop & adopt by rule physician orders for life-sustaining treatment (POLST) form; requires AHCA to act as state clearinghouse for compassionate & palliative care plans; authorizes hospice care team to withhold or withdraw cardiopulmonary resuscitation if patient has POLST form that contains such order; requires DOH to establish procedures for honoring POLST form; requires health care surrogate to provide written consent for POLST form under certain circumstances.
