Health Insurance Restrictions on Prescription Medication Protocols; Requires Medicaid managed care plan, insurer, or HMO to allow prescribing provider to request override of restriction on use of medication imposed through step-therapy or fail-first protocol; prohibits duration of step-therapy or fail-first protocol from exceeding time period specified by prescribing provider.
