Health Insurance Coverage for Prescription Eye Drop Refills; Requiring a health insurance policy that provides coverage for prescription eye drops to provide coverage for renewal eye drops if certain conditions are met; requiring such a policy to provide coverage for one additional bottle of prescription eye drops under certain circumstances; providing that covered prescription eye drop benefits are subject to the same annual deductibles, copayments, or coinsurance established for all other prescription drug benefits under the policy, etc.
