Insurance Coverage for Mental and Nervous Disorders; Requiring specified entities that transact group health insurance or provide prepaid health care to make available to policyholders under specified policies and contracts certain benefits for the care and treatment of mental and nervous disorders without an additional premium; requiring policies and contracts to provide for the transfer of unused inpatient hospital benefits to outpatient benefits or residential treatment benefits, etc.
