Coverage for Skin Cancer Screenings; Requiring individual health insurance policies; group, blanket, and franchise health insurance policies; and health maintenance contracts, respectively, to provide coverage and payment for annual skin cancer screenings performed by a licensed dermatologist without imposing any cost-sharing requirement; specifying a requirement for and a restriction on payments for such screenings, etc.
