The program's goal is to provide the senior population with a coordinated health care and senior services system that is cost-effective, high quality and consumer focused. Another goal of the program is to implement a model that can be replicated throughout the State.

Project Description:
The mission of the Golden Access Partnership for Seniors Program (G.A.P.S.) is to guide Broward County in providing the senior population with coordinated health care and social services that are cost-effective, high-quality, consumer-focused, and that are accessible and easily understood. The program would provide a continuum of services that address the total needs of the senior population. In addition the program will provide measurable outcomes that improve the health and quality of life for Broward County elders. It is expected that the GAPS program will assist in eliminating the confusion and duplication in health and social services that seniors, their families, caregivers and health professionals often face. The GAPS program is based on the Governor's "Five Pillars", which are part of the Governor's Florida Golden Choices program. The Five pillars program addresses: "Aging in Place"- the right place for the right person at the right time; "Aging with Security"-protection from abuse, neglect and other crimes everywhere-at home in the community and in institutional care; "Aging with Dignity"-respectful care for elders for the rest of their lives and for their loved ones; "Aging with Purpose" recognition of elders' contributions of talent and experience to the labor force, volunteerism and younger generations; and "Aging in an Elder-Friendly Environment"-fostering elders' quality of life, safety, and autonomy both at home and throughout the community. The North Broward Hospital District in conjunction with the Area Agency on Aging of Broward County will: 1.Promote successful "aging in place" by offering elders and their families, community-based individualized medical and social services focusing on managing everyday tasks, dealing with illness, and minimizing family care burdens, thereby avoiding premature unnecessary hospitalization and/or premature institutionalization of seniors 2. Enhance public awareness of community resources that are available to assist elders and their families by increasing access to information regarding wellness programs, care-giving services and "aging issues" materials. 3. Implement a replicable model of health care for the elderly to ensure appropriate geographic service locations through a partnership with the Area Agency on Aging in Broward County. The partnership will also include other organizations such as Nova Southeastern University. This model would include educational components, enhanced communication, coordination of services, cost effectiveness, tracking and monitoring and specialized training. 4. Create a system of continuous service evaluation that includes measurable outcomes, time frames, measurable outcomes and data systems. Some of the outcome measurements anticipated are: decrease in hospital utilization due to adverse drug reactions and unnecessary falls; decrease in Medicaid funded medications; reduction in premature nursing home admissions; lower ER utilization; increase in customer satisfaction; enhanced communication between social service and health providers who serve the elderly; and improved medication management

To provide measurable outcomes from the program that demonstrates improvement in the health and quality of life for Broward County elders. To eliminate the duplication and confusion that currently exists for seniors, their families, caregivers, and health care professionals.
