Project Description:
Florida has initiated a statewide effort to deliver more community treatment services for persons with severe and persistent mental illness through Florida Assertive Community Treatment Teams. Thirty teams have been awarded contracts including teams in Tallahassee and Pensacola. Awarding a team to Life Management Center allows the County to adjust for the loss of access to Florida State Hospital and G.P. Wood State Hospital.

Measurable Outcome Anticipated:
Services are provided around the clock by a clinical team of psychiatrists, nurses, counselors and case managers to those persons for whom traditional community services are not able to divert from state hospitalization or jail incarceration. Severnty-five to eighty percent of these services are provided in community settings and not a clinic. Support for hosing, medication and special needs are also included in the services offered. This service would support the state's goal to reduce costs for state hospitalization of severely and persistently mentally ill persons.
