Project Description:
Maintaining 2 dementia specific day service programs within neighborhoods, 15 minutes from a caregiver's home in Palm Beach County. Programs are specifically designed for Alzheimer's disease and related disorder patients that qualify under Alzheimer's Disease Initiative (ADI) program administered by DOEA. Funds will be used for patients who cannot afford daycare expenses and for case management duties in accordance with ADI guidelines. Funds will directly support expenses, including salaries and indirect expenses required to operate programs.

Measurable Outcome Anticipated:
100 patients will be served and receive dementia-specific care through the 2 daycare programs. All patients will be registered with the National Alzheimer's Association Safe Return Bracelet Program; receive daily personal care, medically monitored daily by nurses who specialize in dementia-specific behavior and health care and receive therapeutic activities. Program will delay nursing home placement by 2 to 4 years, entirelyh preventing it for 65% of the program's patients. Family members will receive education, have access to weekly support group, and have information and resources about the disease through ACC's information and referral service. Patients and community will have access to options that are humane and dignified which is a hallmark service for Palm Beach and Martin Counties.
