Project Description:
Serving seniors with diabetes, cardiovascular disease, congestive heart failure, coronary heart disease, and osteoporosis by providing education and careful monitoring, including screening, testing, counseling and medication management.

Measurable Outcome Anticipated:
Seniors in programs experience a dramatic reduction of serious complications from chronic diseases including the prevention of blindness & kidney failure in individuals with diabetes, reduction of heart attacks and strokes in individuals with elevated cholesterol levels, and increased fitness from exercise programs.
