Project Description: |
A critical need exists for these and increased residential services for individuals living with mental illness in Jacksonville. In dividuals with mental illness and/or chronic substance addiction are priority populations for The 2002 Duval County Homeless Continuum of Care. In addition, in Jacksonville, operation " Spot Check" has lead to the closing of many substandard ALFs and boarding homes. This has created a dire situation for those individuals that have absolutely nowhere to go. RRHS ALFs are desparately required to meet this need. The ALF will provide a supportive environment in which clients receive help in getting medical care, attending appropriate treatment and taking medications. Each individual will be linked with appropriate supportive services. Transportation will be provided to necessary appointments. To address medical needs, each individual that moves into the ALF will have an assessment with a River Region nurse, who will obtain information on disease history, medications, general health and medical care. |
