Statewide Interest:
Assisted living, with extended congregate care services, will help meet the need for affordable, long term, quality care. Each resident kept from premature nursing home placement is a potential state savings of $24,000 to $30,000 per year/per resident. The project is also consistent witht eh DOEA move to reverse inappropriate nursing home placement with the mroe desired ALF/ECC alternative living setting. Additionally, the project compliments the Governor's "Destination Florida" initiative to promote Florida as the state of choice for retirees through quality and affordable services. If half of the state eligible residents were to be served by Autumn House ALF, the state pay back would be approximately only tow (2) years.

Project Description:
Okaloosa Board of County Commissioners has leased an outdated county hospital building to Elder Services of Okaloosa County for $1.00/year. This facility is to be used as an Assisted Living Facility for low and modest income, at-risk elderly and/or handicapped individuals. This facility will provide Extended Congregate Care for state recipients as well as private pay arrangements. State support is needed to rehab. the building and bring it into compliance with current codes. Major tasks include: 1) HVAC and Electrical installation, refurbishing, or replacement: 2) Fire Alarm System and Fire Sprinkler Installation; 3) Install Emergency Generators; 4) Repair Elevator; 5) Install drop ceilings and replace damaged tiles; 6) replace 171 windows; and 7) Install carpeting and floor covering. In-kind will be generated by volunteers whol will provide much of the cleaning, scraping, patching, and painting. Private grants and funds are solicited to support the rehavilitation. Restaurant equipment and residential furnishings are routinely solicited and received.

Providing care to 65 at-risk persons in the community at a savings of $24,000 to $30,000 per year/per resident. Preventing, or reversing, premature or inappropriate nursing home placement by providing the ability to "Age in Place." Provide quality cost efficient alternative living situations for elderly or handicapped individuals.
