Project Description:
Initiate and maintain a dementia-specific day center service within a St. Lucie County neighborhood. The service is specifically designed for Alzhiemer's disease and related-disorder patients that qualify under the Alzheimer's Disease Initiative program administered by Department of Elder Affairs. The service will be administered by a provider currently operating 9 other dementia-specific day center services in Palm Beach and Martin Counties. Funds will be used for capital outlay and support expenses, including salaries and indirect expenses required to start up and operate the service, for patients who cannot afford daycare expenses and for case management duties in accordance with ADI guidelines.

Measurable Outcome Anticipated:
100 patients will be served and receive dementia-specific care through the day center service. All patients will be registered with the National Alzheimer's Association Safe Return Bracelet Program; receive daily personal care, be medically monitored daily by nurses who specialize in dementia-specific behavior and health care, and receive therapeutic activities. This service will entirely prevent nursing home placement for 73% of patients, and delay placement by 2 to 4 years for most others. Family members will receive education, have access to weekly support groups, access to 24 hour case management, and have information and resources about the disease through ACC's information and referral service.
