Statewide Interest:
Better prepared individuals at time of disaster and reduction in health disparities. Also there will a reduction in the need for case management services for multi-cultural, isolated, underserved, and low income families. There has been an emerging public health crisis and growing disparity for African Americans and Hispanics as it relates to Alzheimer's disease. Alzheimer's is more prevalent among African Americans than among whites. One study estimates that African Americans are four times as likely to have Alzheimer's than Caucasians. Hispanics are twice as likely to develop Alzheimer's disease. There are approximately 255,000 African American elders and approximately 400,000 Hispanic elders in Florida. Dementia affects 10% of those over age 65 and 50% of those over age 85.

Project Description:
Reducing the need for case management services for multi-cultural, isolated, underserved, and low income families. Educating families on services, support, and disaster preparedness. Education is the key to quality of life in home and community based settings for the person coping with Alzheimer's. The very successful "Multi-cultural Outreach" program provides Alheimer's support, education, and services through community collaboration, 24/7 bilingual telephone support, outreach and public education to the underserved, multi-cultural families throughout Florida. COMMUNITY COLLABORATION: Increase and enhance community outreach, network and awareness. Meet and coordinate projects with local AAAs, Memory Disorder Clinics, Research Institutions, and STARS and Beyond to ensure all staff is aware of available Alheimer's resources within each agency. 24/7 BILINGUAL SUPPORT: Increase and enhance support services, including a multi-language, 24/7 telephone support. Coordinate with 211 providers to ensure that Alzheimer's resources are known to them. OUTREACH: Reaching out ONE-ON-ONE to African Americans and Hispanics. Work with local clergy to identify possible people with Alzheimer's. Education and training on local services and support. Increase awareness on disaster preparedness including hurricane response. PUBLIC EDUCATION: Present, speak and train individuals and groups to increase awareness and knowledge of Alzheimer's disease. Develop a culturally appropriate awareness camapign through faith-based and other community organizations that includes key leaders, clergy and members of the targeted communities.

Identify and target underserved communities and provide appropriate support services outreach, and awareness. Establish local organizations or key contacts to connect minority leaders and key service providers. Develop relationships with faith communities, civic leadership and community organizations that serve the target groups. Develop and implement marketing/outreach strategy. Increase the awareness of dementia services available to the target audience. Increase the access of services available to the target audience.
