| Project Description: |
| Vocational placement and follow up is a major component of the emerging treatment philosophy of resiliency and recovery. Current D 4 general revenue funding levels and Medicaid billing guide lines preclude meeting growing service demand and do not allow for significant follow up \ ongoing activity to insure continuing vocational success. This funding request would allow for the expansion of job readiness \ preparation, job development \ placement, and sustained follow up via case management \ individual counseling. The target group would be people being released from state hospitals, forensic facilities, and transitioning out of DCF foster care. It is estimated that approximately 250 will be served on an annual basis. |
