Project Description:
Bridges of America's 100 Bed Post-release Transitional Housing Program provides therapeutic support, affordable housing, job placement, assistance, and additional substance abuse treatment through referrals when needed. The program links services to offenders and inmates who are rleased from communit7 and prisons. We also provide and link services to offenders who are currently on community supervision or have been released from prison or community supervision within ninety, (90), days and have probation to foll9ow and is assessed to need transitional housing services. The program is designed to meet the various needs offenders and inmates experience when attempting to reenter their communities. The case managers at each program location are responsible for the inital intake and assessment to determine what services are essential for the resident reinegration back into their communities. The overall mission of the Post-release Transitional Housing Program is to assist post-released offenders by providing transitional housing, an dother such support services to assist them in maintaining a durg-free, independent living, crime-free life style and make a successful re-entry into the community.

Measurable Outcome Anticipated:
Bridges of americas 100 Bed Post Release Transitional Housing Program houses approximately 525 residents annually, of this number 60% were successfully transitioned scucessfully back into society from those transitioning from community based tratment programs. 20% sucess rate from those transitioning from prison. Substance abuse tests were conducted in the amount of 1560 tested with only a 3% testing positive. The average program stay is three to four months.
