Project Description:
Electronic Health Records improves care, delivery and quality. Our health centers must be prepared to participate in the electronic change of data in order to properly assess the effectiveness and outcomes of healthcare delivery expected by the state. Services: A multitude of comprehensive, quality primary and preventive care provided to residents in three counties. CFHC participates in the Diabetes Collaborative disease management program. The Cardiovascular Collaborative disease management program will commence early October, 2007.

Effective integration between personal health and electronic records. Improved operations: 1)streamlining of charting practices, 2) storing of information eletronically, and 3) searchable database records to prevent potential hazaards of handwritten files.
