This program empowers the family as a whole by addressing the health, socio-cultural, socio-economic and educational needs of each member of the family unit through heavy emphasis on prevention, maintenance of positive behavior changes,service coordination and collaboration with other community organizations with proven records of success. This program also assists other community based organizations with capacity building to better accomplish their respective objectives.

Project Description:
This program addresses and helps improve the care and quality of life for the entire family through a multidisciplinary educational approach and development of forums to foster education management and capacity building for other community based organizations.

Increased access to Heath Care; increased health literacy and participation in one's family's health care management; reduced Health Care Disparities; reduced truancy through effective afterschool programs and mentorship; increased parental involvement/participation in K-12; increased effectiveness of community services through effective management, capacity building and coordination of services; increased family safety.
