As a result of a shortage in nursing home beds and increased senior long term costs these dollars will reduce the state costs by allowing seniors to age in place. Aone percent increase in the number of well elders would decrease Medicaid expenses by 33 percent per year.

Project Description:
JFCS seeks to continue to provide services to seniors in the State of Florida through its NORC program (Naturally Occurring Retirement Community) This program is in direct response to Governor Bush's "GOLDEN CHOICES" Blueprint developed in it's "Communities for a Lifetime." The blueprint emphasizes: 1. Aging in Place- The right of Floridians to age in the Communities of their choice in the least restrictive environment. 2. Aging with Security- The right to live without fear of abuse, neglect, or any other crimes. 3. Aging with Dignity- The right to live with dignity and respect. 4. Aging with Purpose- The right to contribute talent, experience or economic strength to the community. 5. Aging in an Elder Friendly Environment- The right to participate in a community that fosters elder's quality of life, safety and independence both at home and throughout the community. The blueprint encourages collaboration with state, local, agencies and faith based entities to help fill service gaps. The NORC program fulfills all of the requirements of the plan. JFCS has collaborated with USF, Senior Friendship Center, Catholic Charities and other area agencies to provide a study of area needs as well as services that are necessary to help seniors age in place. The NORC program has focused in North Sarasota where there are gaps in services as well as concentrated seniors who desire to age in place. The plan is to expand the services to the South County including North Port as well as Manatee County. The services provided are targeted to break isolation and to identify the needs of seniors. The NORC in Sarasota has expanded to other concentrated senior populations that live in mobile parks and where studies have identified other large concentrations of seniors. NORC program has provided a singular opportunity to deliver targeted social services and supportive services cost-effectively; increase service availability; crises intervention; develop neighborhood resources; and organize neighborhood stakeholder councils consisting of seniors that live in the communities. The outcome will be to empower seniors, create community, develop partnerships, deliver a "basket of services", maximize existing services, identify needy seniors, educate the community and encourage and utilize volunteers both intergenerational and by the seniors themselves. This program prevents premature admission to nursing homes and hospitalizations thereby cutting the costs to governmental agencies. SERVICES TO BE PROVIDED: � JFCS will continue to train Senior Advocates and Social Workers to organize the seniors in the neighborhoods to develop a "neighbor to neighbor" concept under which seniors will be trained to take ownership of their community and assist in developing stakeholder councils, made up of senior leaders in the community. Programs will be presented in the community to provide information in identifying at risk seniors and to provide wellness programs. Case management and financial aid will be provided to assist in crisis situations. Caregiver training and support activities will be promoted. Volunteer activities including intergenerational programming will be promoted. Enhance interagency coordination and cooperation by continuing advisory committees. JFCS will provide Mental Health and care manager programs In collaboration with local housing authorities, expand supportive services in subsidized complexes and other communities with large concentration of seniors. Promote use of environmental modifications and assistive technologies to facilitate the independent living of frail elders Increase the collaboration of faith based entities such as nurses in providing volunteer activities As a result of providing senior services to the community we have encountered increasing serious and interrelated problems that include the following situations: Elders living in unfit environments that include lack of food, unsanitary conditions, and unmet physical and emotional needs. Unmet medical needs Social isolation leading to depression and chronic illness Lack of social supports Seniors existing from crisis to crisis due to limited resources Feelings of hopelessness and a lack of options

Measurable Outcome Anticipated:
1. There will be an increase of engagement in activities outside of the home by at least one external activity a week. 2. There will be an increase of personal interaction with neighbors and/or community by phone or in person at least 2x's a week. 3. Individuals will remain in their home by assessing that 50% of identified seniors in the program will age in place. 4. 100% of seniors in the program will be linked to community and senior resources should there be a need. 5. 80% of seniors attending education programs will exhibit increased knowledge as to services and senior needs. 6. 2,000 seniors will be served that are currently not being served
