| Project Description: |
| Community Health of South Florida, Inc. (CHI) currently provides primary care related services located in the Public Health Department's West Perrine Health Center, serving the uninsured and underinsured population of Perrine, Florida and South Miami-Dade County. This project seeks to develop existing space within the health center into four new exam rooms complete with indiviual sinks and cabinetry. The West Perrine Exam Room project has many benefits which include aiding in eliminating barriers to care, increasing patient capacity and decreasing patient waiting time. A goal of this project is to help create happier, healthier residents of Miami-Dade County. Architectural plans developed by Robert Barnes and Associates is attached. |

| Currently, CHI serves 670 visits per month at the West Perrine Health Center, utilizing two exam rooms. CHI projects that four additional exam rooms will increase pation visits at the West Perrine Health Center to 2,000 visits per month within a year after completion of the project. |
