Statewide Interest:
The Florida Department of Elder Affairs' funding provided through the Lead Agency grant ($ 1.6 million) covers the costs of serving 437 seniors, but does not meet the needs of Palm Beach County overall and very specifically South Palm Beach County. Almost double the numbers of seniors, 1035 are on a waitlist. This waitlist of elderly who have been screened for services but cannot then have their needs appropriately assessed and met, grows yearly, not only because of inadequate resources which keep people on the wait list, but also an increase in the life span of older seniors (85+ group). The greatest concentration of frail elderly, homebound elderly, and older seniors live in the RRJFS service area. Of the seniors in south Palm Beach County who request services, slightly over one-third are being served leaving a substantial population of individuals who are at risk of being removed from their home setting if their condition deteriorates and they can no longer fulfill activities of daily living without support. It is this grouping of seniors that we are requesting additional funds to serve. We are asking consideration for South Palm Beach County, where the data and demographics highlight that a significant sector of our aged residents requires assistance. Project Description:
To provide additional case management and services to address the needs of those seniors who are "pending" on a waitlist, through the lead agency. Through this funding three case managers and one case aide will be hired to provide assessment and oversight of service delivery to consumers primarily focusing on those people currently on the waitlist. The remaining funds would be used to provide direct services to the clients. Services may include: adult day care, chore, companionship, counseling, emergency alert response, escort, homemaker, medical supplies/services, personal care, respite, shopping assistance, and/or meals. Existing vendors for the Community Care for the Elderly (CCE) program will provide the designated services. CCE consumers presently receive approximately $ 3,000 per year in direct services (excluding case management/case aide expenses)

Measurable Outcome Anticipated:
It is the anticipated outcome that these additional dollars will facilitate a 25% reduction on the waitlist. The case managers will triage those with a lower priority status in order for them to access necessary services quickly, as well as those at the higher priority status so that their condition can be stabilized. In this regard, severity of case mix will be adjusted thereby addressing both our most at risk seniors, as well as those who we can give a less intensive allocation of resources because they have a greater degree of self-sufficiency. Addressing those people who are at less risk will serve as preventative care, hopefully getting these people's needs met before they become high risk. Due to the current state requirements these people never receive services. Our goal with this program that we have named, SLOW, Seniors Languishing on Waitlists is to SLOW to Fast for Fast access to services today.
