Project Description:
TGNI Precision Care is seeking financial assistance from the State of Florida to provide Home Care services to the frail elderly individuals that are physically and mentally challenged underserved/ uninsured in Miami-Dade County the funding will be used to expand TGNI Precision Care�s services to the underprivileged areas in the county which have been identified as having difficulties of their Activities of Daily Living (ADL�s) and their Instrumental Activities of Daily Living (IADL�s). Based on the Department of Elders affairs (DOEA) report on the Miami-Dade County Level Data, it indicates that the population of Elders that are 60+ are more than 414,608. More than 86,923 (20%) in poverty, 94,148 (22.7%) are low income < 125% of poverty level, 30,661 (7.4%) are mobility Self -care limitation, 81,806 (19.7%) that are living alone and 45,520 (10.1%) probably suffer from alzheimer�s disease and only 37,530 that are enrolled in all DOEA Programs combined. Services to be provided included but not limited to Screening/Assessment, Companion, Homemaker, Personal Care, Escort, Shopping Assistance, Tel Reassurance and In-Home Respite. Home Health Services are required because of the many complexes medical issues facing our clients. Long-term care has changed radically in the past few years. While the populations are living longer and better quality of life, while other individuals are still succumbing to the many health complications. These complications require medical interventions in both the in-and- out patient setting. Many of these needs can only be met in the home via home health care. The single biggest benefit the home care program provides is access to health care for low- income elderly and individuals that are mentally or physically challenged. The greatest numbers of hours of services we provide are to minority clients. Many of these clients are living alone. Obviously, in many of these cases, having regular visit to their home is literally a matter of life and death. In other cases, the home care program helps individuals to maintain their health or to prevent health problems. The regular access we provide to medical checkups, pharmacies or the grocery stores are invaluable to clients who are unable to run errands that most of us take for granted. In addition, using volunteers creates a neighbor-helping-neighbor program that gives people hope and support as they face aging, disability and isolation. When clients thank us, they often stress the emotional boost they received from a kind caring volunteer. Nurses and support staff that understand the linguistic and cultural diversities of the clients, which we serve. Services should be tailored to elder clients and their specific needs including hearing, vision, mobility, memory and other considerations. The respective catchment�s area that we indicated is a multi- cultural population area with cultural differences and nuances and needs to be treated holistically by an agency which can reinforce the understanding and respect of cultural and linguistic values of everyone. Our main target population is the Haitian-American Community, however because of the demand for services we also service other communities. The geographic area that we serve is from 36th St South to 215th St North and from 15th Ave. North East to 42nd Ave North West, that includes Little Haiti, Opa-Locka, Carol City, Miami Gardens, North Miami and North Miami Beach just to name a few. Demand for the home health care service has increased dramatically as the elderly population of our target area has increased tremendously. Over 23 % of all home care patients are referred by case managers and physicians. Those clients that are sick and homebound require these Home Health Care services. Clients to be served are those clients being discharged from Miami-Dade county area hospitals and those who can demonstrate needs for services. Gender and ethnicity is unknown, but should reflect the demographic of the un- and under-insured. Services are provided to clients requesting services without regard to sex, sexual orientation, race, religion, nationality and ethnicity.

Measurable Outcome Anticipated:
With the funding requested, TGNI Precision Care will be able to provide the best continuous home care services to enhance the lives of 20 frail elderly individuals that are physically and mentally challenged and to enable them to be aged in place with dignity and with a purpose recognizing the cultural diversity of the community that we serve, it is estimated that 4 to 6 hours/ units of services will be delivered a week based on client�s needs, these services are as follow: Screening/Assessment, Companion, Escort, Homemaker, Personal Care, Shopping Assisstance, Telephone Reassurance and In- Home Respite.
