Industrial Life Insurance Policies; prohibits delivery or issuance of industrial life insurance policies after certain date; provides application; requires disclosure of certain information to policyholders or premium payors; deletes application exception from certain secondary notice requirements. Creates 627.5015; amends 627.4555,.5045.
