Health Insurance Policies Eye Exams; requires health & group health insurance policies to provide coverage for certain eye examinations & medically necessary followup visits for specified minors; requires health maintenance contracts that cover certain minors to provide coverage for certain eye examination & medically necessary followup visits. Creates 627.6414; amends 641.31.
