Pharmacy Benefits/ID Cards; provides definitions; requires administrators of certain insurance plans to issue identification cards to covered individuals; specifies required information; prohibits pharmacy benefit managers from selling lists of patients which contain certain information; provides exceptions & application; requires certain health benefit plans to issue identification cards to plan enrollees; specifies required information, etc.
