Workers' Compensation/Disability; substantially revises provisions governing compensation for disability; provides guidelines re permanent total disability, temporary total disability, permanent partial disability, temporary partial disability, & subsequent injury; provides consequences if employee refuses or leaves employment; substantially revises provisions re attorney's fees & costs; provides penalties for violations, etc. Amends 440.15,.34.
