HMO Contracts/Health Care Providers; requires AHCA to establish statewide health care provider & health maintenance organization qualification dispute resolution program; requires agency to contract with resolution organization for certain purposes; revises provisions providing for termination of provider contract by HMO; prohibits certain insurers from refusing to enter into or renew contracts with certain health care providers, etc. Creates 408.7058; amends 627.6474, 641.315.
