Health Care Advance Directives; revises definition of "incapacity" or "incompetent"; clarifies requirements on palliative care; eliminates requirement for written notification to health care surrogates by health care facility; clarifies that surrogate cannot override principal's wishes as expressed in living will; requires Driver Licenses offices to make forms available to public; authorizes certain individuals or business entities to act as proxy, etc. Amends Ch. 765.
