Congenital Craniofacial Anomalies; requires individual health insurance policy or group health insurance policy to cover services needed to treat said anomalies as authorized by physician; includes within exclusion from provisions applying to out-of-state-groups group policies providing health coverage for said anomalies; requires contract by HMO to cover services needed to treat anomalies as authorized by physician. Creates 627.64194,.6614; amends 627.6515, 641.31.
